Understanding the PHQ-9 Depression Questionnaire

What the nine questions measure and what the score means.

· 2 min read

What it is

The PHQ-9 asks how often in the last two weeks you were bothered by nine symptoms of depression. Each is scored from 0 to 3.

Interpreting scores

Scores of 5, 10, 15 and 20 mark mild, moderate, moderately severe and severe symptoms. A score of 10 or more is a common threshold for further assessment.

What to do next

It is a screening tool, not a diagnosis. If you score high or have thoughts of self-harm, contact a clinician or crisis line. In the US, call or text 988.
